Summary:
Samaritan Hospital is seeking an experienced Outpatient Social Worker to join our dynamic team.
The ideal candidate will be responsible for providing comprehensive social work services to patients and families in our outpatient setting.
The successful candidate will have a strong background in clinical social work, experience working with diverse populations, and a commitment to delivering high-quality patient-centered care.
Responsibilities:
Conduct comprehensive psychosocial assessments of patients and families.
Develop and implement treatment plans with patients and families to address their individual needs.
Provide crisis intervention and support to patients and families in times of stress and difficulty.
Facilitate patient and family education to promote self-advocacy, independence, and coping skills.
Collaborate with healthcare providers, community agencies, and other stakeholders to ensure continuity of care for patients and families.
Participate in interdisciplinary treatment team meetings, case conferences, and discharge planning meetings.
Maintain accurate and up-to-date documentation of all patient interactions and interventions.
Adhere to ethical and legal standards of practice, including maintaining confidentiality and reporting suspected abuse or neglect.
Requirements:
Master's Degree in Social Work from an accredited program.
New York State Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) required.
Limited Permit Accepted Minimum of two years of experience in a clinical social work setting.
Strong communication, collaboration, and problem-solving skills.
Demonstrated ability to work effectively with diverse populations, including individuals with mental illness, substance use disorders, and complex medical conditions.
Knowledge of community resources and services for patient referrals.
Ability to work independently and as part of a team in a fast-paced environment.
Experience with electronic health records preferred.
